CTC Hike : On the Previous Package 30% to 40% Hike.

It is an intricate workflow involving certain checkpoints that every claim must go through before it’s approved. If the claim makes it through all these checkpoints without any issues, the insurance company approves it and processes any insurance payment.
Hospital Claim –Billed in UB04 Form per CMS.
Medical Claim – Billed in either CMS 1450 or CMS 1500 Forms per CMS
